IN ORDER TO BECOME A FULLY-ENROLLED AGENT WITH MCGOWAN ALLIED, WE REQUIRE THE FOLLOWING ITEMS:
  1. Broker Questionnaire
  2. Brokerage Agreement
  3. Updated W9 Form
  4. P&C Licenses (Agency & Individual) for each state in which you do business;
  5. Declaration Page from your E&O Policy.
